Care Navigator

Job Details: Full-time position working in a variety of settings including working in the office at the Region V Systems and with various community partners.   Form relationships with the Department of Corrections and other community providers to assist program participants to find and utilize local resources.  Position requires driving to connect with participants and resources throughout the community.  You will assist the participant in identifying their needs, developing wellness plans, and following up with them to assess further needs.

Responsibilities and Duties:

·       Treat all participants with dignity and respect, consider participant needs, and advocate for the people we serve

·       Maintain confidentiality of agency, participants, employee, and salary information.

·       Promote the human dignity, rights, safety, and well-being of program participants

·       Ability to advocate for the needs of the participants

·       Demonstrate good communication with program participants, supervisors, peers, outside resources.

·       Submit paperwork and adhere to deadlines as assigned

·       Able to communicate and form relationships with participants through sharing lived experiences, find and utilizing local resources and develop relationships with community partners.

·       Ensuring data quality, accurate tracking, and reporting of engagements

·       Always maintain the highest level of work and professional ethics

·       Complete on-going job-related training as required by the position

·       Position will contain medium physical work consisting of exertion of up to seventy (70) pounds of force occasionally exerting up to twenty (20) pounds of force frequently and exerting up to ten (10); pounds of force often to move objects. This position consists of a combination of sitting, standing, or walking

·       Other duties as assigned


·       Ability to be approved to enter the Department of Corrections.

·       Ability to drive personal vehicle to provide transportation and supports to participants

·       Ability to maintain professional connections with program participants, supervisors, peers, outside resources.

·       Ability to multi-task, work independently and as a team, and prioritize job responsibilities.

·       Proficient in technology including computers, phones and multiple computer programs including but not limited to outlook, word, excel, and OneDrive.

·       Strong oral and written communications.

·       Strong analytical, problem solving, and organizational skills

·       Demonstrate working knowledge of recovery and wellness philosophy as it pertains to substance use, mental health and/or incarnation

Required Skills:

·       Excellent interpersonal skills and the ability to work well with staff across a variety of positions and levels of seniority throughout the agency and in the field

·       Superior written communication and analytical skills

·       Strong organization skills including the ability to plan, coordinate, and implement plan to assist participants

·       Must be able to think on your feet and the ability to use critical thinking in the moment

·       De-escalation skills

·       Be self-motivated

·       Bilingual is preferred, is not required

·       Valid Driver’s License, insurance, and vehicle

Work Environment:

·       Direct contact with participants through various means including but not limited office, a variety of facilities, and community partners

·       Sitting for extended periods of time, regularly talking to, and active listening to others

·       Estimated travel - 80% of the time
